The Futures First Care Manager (CM) will support Opportunity Youth and young adults ages 16–30 enrolled in the Futures First program. The CM will focus on whole-person care, connecting participants to workforce, education, housing, and mental health resources to help them achieve stability and independence. The Care Manager will conduct intake assessments, develop individualized service plans, provide career readiness support, perform housing inspections for youth housed with the APF and ensure wrap-around linkages through trauma-informed, culturally responsive care. The CM works collaboratively with outreach managers, case managers, and workforce partners to ensure each participant progresses toward employment, education, and personal development goals.
