About The Position

Requirements

  • A background in social work or a related field is preferred.
  • Experience with crisis management and intervention techniques is essential.
  • Familiarity with Medicare processes and child welfare systems is advantageous.
  • Skills in supervising and mentoring staff or volunteers are beneficial.
  • Previous experience in addiction counseling or working with diverse populations is a plus.
  • Strong communication skills with the ability to build rapport with clients from various backgrounds.

Responsibilities

  • Conduct comprehensive assessments to identify client needs and develop individualized care plans.
  • Utilize motivational interviewing techniques to engage clients and encourage positive behavior changes.
  • Provide ongoing support and monitoring of client progress, adjusting care plans as necessary.
  • Collaborate with multidisciplinary teams, including social workers, healthcare providers, and community resources.
  • Facilitate access to services such as Section 8 housing assistance, mental health resources, and addiction counseling.
  • Supervise and mentor junior staff or interns in best practices for client care and crisis management.
  • Implement crisis intervention strategies to support clients in high-stress situations.
  • Maintain accurate documentation of client interactions, progress notes, and service referrals.
