The Care Manager position will focus on provision of person-centered services and comprehensive care management including planning and coordination of all physical, behavioral and support services to increase independent living skills and positive community involvement for enrolled participants. The Care Manager does this through guiding person-centered goals and facilitating meaningful connections to natural and formal support networks. The Care Manager will participate in ongoing professional development to enhance knowledge of community resources, process to support access of services and benefits, and to refine treatment documentation and approach. The Care Manager position operates primarily in the community, while the Care Manager should also be skilled at working at the agency office and virtually via telehealth. The Care Manager position utilizes electronic health record to document work on a daily basis and should be experienced and efficient working within electronic systems.
