About The Position

The Care Manager for Residential Substance Use Disorder (SUD) Treatment Programs is responsible for delivering comprehensive care management services to individuals engaged in residential treatment. This role ensures clients have coordinated access to medical care, behavioral health treatment, pharmacological services, community resources, and age-appropriate resiliency and recovery support services. The Care Manager is an integral member of the multidisciplinary treatment team and actively supports person-centered, culturally responsive, trauma-informed care.

Requirements

  • Master’s Degree in a human services related field OR
  • Bachelor’s degree in a human services related field and 1 year relevant experience working with the target population preferred OR
  • High School Diploma or GED with a minimum of 3 years of experience working with the target population
  • Valid driver’s license
  • Strong understanding of substance use disorders, harm reduction, recovery models, and co-occurring mental health conditions.
  • Knowledge of community resources, social service systems, and healthcare navigation.
  • Ability to build rapport, collaborate effectively, and maintain professional boundaries.
  • Strong organizational skills, follow-through, and ability to manage multiple priorities.
  • Cultural humility and ability to deliver services with respect to diverse identities and backgrounds.
  • Proficient in documentation, electronic health records, and coordinated communication across providers.

Responsibilities

  • Connect clients to appropriate community supports, recovery resources, medical providers, housing supports, employment/education services, and other social determinants of health resources.
  • Ensure timely coordination and linkage to medical, behavioral health, and pharmacological services, including MAT providers when applicable.
  • Monitor continuity of care throughout the treatment episode and during transitions between levels of care.
  • Serve as an active member of the treatment team, contributing to treatment planning, case consultation, problem-solving, and discharge planning.
  • Recognize and address the complex needs of individuals with co-occurring mental health and substance use disorders (dual diagnosis).
  • Engage clients using motivational, strengths-based, and recovery-oriented approaches to build rapport and support treatment goals.
  • Provide culturally appropriate, person-centered services that respect and reflect client identity, background, and lived experiences.
  • Conduct ongoing outreach for clients who disengage, ensuring attempts to re-engage are timely, documented, and aligned with program expectations.
  • Coordinate with internal departments (clinical providers, peer support, medical staff, residential staff, etc.) to ensure integrated service delivery.
  • Communicate and collaborate with external partners, such as hospitals, outpatient providers, probation/parole, social services, and community agencies, to facilitate seamless care.
  • Complete timely, accurate, and regulatory-compliant documentation, including assessments, service notes, referrals, care plans, progress updates, and discharge summaries.
  • Maintain confidentiality and adhere to all regulatory standards, including HIPAA, 42 CFR Part 2, state regulations, and internal policies.
  • Engage in cross-training activities with other team members.
  • Attend and participate in supervision sessions and demonstrate the ability to apply supervisory recommendations to work performance.
  • Additional responsibilities as needed to support program operations.
