About The Position

The Care Manager serves as the primary point of contact and advocate for families after a student’s initial enrollment period. This role is central to care coordination, communication, and collaboration across clinical, residential, academic, and administrative teams to ensure a supportive, seamless treatment experience.

Requirements

  • Must be at least 21 years of age
  • 1+ year of experience in a residential treatment or behavioral health setting
  • Strong communication, organization, and collaboration skills
  • Ability to pass background check

Nice To Haves

  • Bachelor’s degree in a related field preferred with a strong preference for Bachelors of Social Work (BSW)
  • Experience working with families, youth, or clinical teams strongly preferred

Responsibilities

  • Act as the primary liaison for parents, school districts, and referents; provide proactive, timely communication and support.
  • Coordinate and support Treatment Team Meetings, documentation, and interdisciplinary communication.
  • Support new student and family onboarding and ongoing transition processes.
  • Provide logistical and administrative support to the Clinical Department and assist with outcomes tracking and documentation.
  • Build and maintain trusting relationships with families, internal teams, and external partners.
