In this role, you’ll provide intensive community case management, connect members to vital resources, and empower them to adopt healthy behaviors. Your work will strengthen access to care, improve health outcomes, and reduce healthcare costs through proactive outreach and compassionate engagement. As a Care Manager, you’ll collaborate with hospitals, emergency departments, and community organizations to ensure safe transitions and remove barriers to care—making a real impact where it matters most.
