About The Position

The Care Manager plays a crucial role in the care team, responsible for delivering services to clients by supervising caregiver activities. This position involves training, developing, and supervising caregivers to ensure high-quality care for clients. The Care Manager also manages incidents, complaints, and injuries, acting as a backup supervisor when necessary, while supporting company initiatives to maintain operational quality and excellence.

Requirements

  • High School Diploma or GED with two years of full-time study at an accredited college or two years of supervisory experience in a healthcare setting.
  • Excellent time management and organizational skills.
  • Knowledge of applicable healthcare laws and statutes.
  • Skills in early identification of issues and problem-solving.
  • Strong interpersonal communication skills.
  • Supervisory and decision-making skills, adaptability, and positive leadership qualities.
  • Strong written and oral communication skills.
  • Valid driver's license and a legally registered vehicle with minimum liability insurance.
  • Humble, teachable, and open-minded attitude.

Responsibilities

  • Supervise caregiver activities to ensure quality service delivery.
  • Train and develop caregivers to provide the highest level of care.
  • Manage incidents, complaints, and injuries related to client care.
  • Conduct client visits for supervision and evaluation of services.
  • Prepare and maintain caregiver work schedules and necessary documentation.
  • Monitor adherence to caregiver work schedules.
  • Manage a client caseload using Electronic Health Records (EHR).
  • Identify and address client concerns with empathy.
  • Supervise caregivers in performance evaluations and corrective actions.
  • Assist in recruiting qualified caregivers as needed.
  • Act as a backup caregiver when necessary.
  • Perform community outreach on behalf of Outreach Health.
  • Serve as a liaison between Managed Care Organizations and Outreach to improve client care.
