Cayuga Centers is hiring immediately for a Care Manager in Auburn, NY. This role is crucial in supporting children, youth, and families by helping them build strong, life-changing relationships and avoid hospitalization or placement. The Care Manager will be part of the Children's Home Health Care Management team, which focuses on strengths-based, youth and family-centered work. The services provided are designed to offer goal-directed support through a comprehensive, family-driven Plan of Care. The Care Manager will assist in coordinating services for children and families in their homes and communities, referring them to programs that address behavioral and physical health needs. This position offers opportunities to expand skills in assessment, evaluation, and service coordination while supporting program growth within the agency and community. The Care Manager will provide outreach and enrollment services to children presumed eligible for New York State's Children's Health Home program, gathering assessments and entering documentation to confirm eligibility and identify areas requiring support services. In this role, the Care Manager will work closely with children, families, and service providers to create a comprehensive Plan of Care, ensuring the ongoing involvement of families and service providers in executing the plan. The typical schedule is Monday to Friday, 9 am to 5 pm, with flexibility to meet the needs of youth and families, and a rotating on-call schedule. This position requires building trusting relationships with families, providing health home care coordination services, educating families about services, documenting case activities, and assisting families in overcoming barriers to access services.
