About the position

The Care Manager at Loyola University Chicago plays a crucial role in the Wellness Center by participating as part of the clinical team. This position involves managing intake, triage, and urgent care/crisis systems, as well as providing consultation regarding community referrals. The Care Manager assists students in accessing outpatient mental health care services, coordinating with various campus personnel and community resources. Responsibilities include developing follow-up systems for students referred to external providers, assessing insurance benefits for mental health treatment, and supporting students transitioning back to campus after hospitalization. The role also involves maintaining records of care management activities, cultivating relationships with off-campus mental health providers, and participating in staff meetings and training activities.

Responsibilities

  • Participates as part of the clinical team, providing assistance with managing intake, triage and urgent care/crisis systems.
  • Assists and supports students in accessing services from community resources for outpatient mental health care.
  • Develops and implements systems for follow-up with students referred to community resources.
  • Follows up with external providers to ensure clients are appropriately assessed and compliant with treatment recommendations.
  • Assists students who have been hospitalized for mental or physical illness with discharge planning and follow-up care.
  • Collaborates with counseling staff in treatment and disposition planning for students with complex needs.
  • Consults with parents, faculty, and administrators to ensure quality care for students.
  • Maintains appropriate records and documentation of care management activities.
  • Cultivates strong relationships with off-campus mental health providers and community partners.
  • Maintains a comprehensive database for mental health providers in the Chicagoland area.
  • Attends and participates in Wellness Center meetings and training activities.
  • Assists with mental health phone coverage.
  • Might carry a small brief counseling case load during peak demand.
  • Pursues professional development in service of the responsibilities of the position.

Requirements

  • Master's degree in social work, counseling or related mental health discipline.
  • Current valid licensure in area of mental health specialization (e.g., LSW, LCSW, LCPC) is preferred.
  • Proficiency in basic word processing.
  • Experience with using an electronic medical record system is preferred.

Nice-to-haves

  • Applicants who are eligible for licensure but require supervised clinical hours for licensure may be considered.
