EveryMind is hiring a Care Manager / Youth Development Specialist to join our Bridge to Wellness team at Walter Johnson HighSchool. This uniquely blended role is responsible for 20 hours per week of Care Manager duties, and 20 hours per week of Youth Development Specialist duties. As a Care Manager, you would be responsible for direct case management services within a school and community-based programs designed to integrate a system of resources and services for youth and families and to support them in achieving their goals. As a Youth Development Specialist, you would be responsible for identifying client needs and providing trauma-informed, culturally relevant, creative, and engaging in-person youth development services to high school-aged youth/adolescents. These activities are designed to support mental health wellness and provide mentoring, coaching, and advocacy. Requirements

  • Strong Personal Resilience and Mental Health: Given the high-stress nature of the role, it is essential that candidates possess a high level of personal resilience and maintain stable mental health. This ensures that they are well-equipped to provide effective support and maintain the professional standards required for client care.
  • Education: Bachelor’s degree in social work, psychology, social services, or a related field required.
  • Experience: A minimum of 1 year of relevant social/emotional wellness experience working with youth/adolescents and families. Experience providing community school or social services programming preferred.
  • Skills: Strong verbal and written communication. Effective organizational, community building, and record keeping skills.
  • Abilities:Effectively interact and work collaboratively in a flexible work environment with a variety of constituents including clients and family members or caretakers, interns, volunteers, paid staff, CEO, board members, and community partners. Ability to express ideas with clarity, confidence and sensitivity. Must be able to maintain confidentiality involving clients in accordance with organizational and federal guidelines.

  • Clearance Requirements: The employee must pass a criminal background check. Certain job functions may require employee to pass a child sexual abuse and other criminal activity as well as a credit check. Responsibilities

  • Completes comprehensive needs and strength assessments with youth and families resulting in individualized service plans and goals and appropriate crisis intervention or support as needed.
  • Consults and collaborates effectively with colleagues, school staff, and other County/community agencies to provide comprehensive services for clients. Offers information and referral support for community members, school staff, etc.
  • Provides on-going social services to clients which may include securing needed resources, providing necessary referrals, and ensuring any court-ordered requirements are met. Assists with completing applications for programs and services that align with service plan goals.
  • Maintains compliance with School and Community-Based Youth Services contract including: a. Meeting productivity requirements b. Maintaining minimum caseload requirements c. Timely submission of data through County electronic records system
  • Conducts psycho-educational groups, educational workshops, and resource presentations as appropriate.
  • Maintains accurate, thorough, and timely documentation of client records. Documents and evaluates services provided and client progress, updates service plan at regular intervals.
  • Manages funds in assigned budget categories and utilizes assigned client needs funds to support service plan and client goal achievement.
  • Assists with seasonal events, such as camp registration, school supply drive and holiday giving program.
  • Provides translation and interpretation services for clients and others as time allows.
  • Assists with orientation and training of new staff.
  • Utilizes regular supervision with Manager to evaluate and improve the quality of case management services and ensure that services are culturally appropriate for the client.
  • Maintains client confidentiality according to EveryMind Confidentiality Policy.
  • Adheres to EveryMind and Youth and Family Services Division (YFS) policies established by EveryMind Board of Directors, Chief Executive Officer, Chief Program Officer, and Division Director.
  • Consistently represents EveryMind and the SCYS program in a professional manner. Treats all students, families, community members, school personnel, partners, donors, teammates and others with respect.
  • Other duties as assigned.
  • Recruits and retains eligible youth and families to participate in school day, after-school. and community programs.
  • Plans and facilitates in-person activities and services when youth/adolescents are available including afternoons and evenings.
  • Plans and facilitates one-on-one and small group activities and services designed to support mental health wellness and provide mentoring, coaching, and advocacy. Activities may include sports, outdoor recreation, gardening, community service projects, performance arts, visual arts or other youth-oriented activities.
  • Incorporates evidence-based practices and individualizes activities to support students with a variety of physical, emotional, and social needs.
  • Conducts regular outreach and collaborates with community partners including MCPS, DHHS, and others to identify community needs and students who would benefit from services, and to engage students and families at time of referral to services.
  • Identifies and coordinates with other recreational and community partners for effective, relevant programming.
  • Conducts regular assessments of students’ needs, incorporating input from key stakeholders including school administration and teachers, parents and caregivers, students, and other essential members of the school community.
  • Implements evaluation aspects of all program components for youth and families. Analyzes outcome and process evaluations and makes service adjustments accordingly.
  • Offers services that will promote family bonding by facilitating joint activities with other family members on a regular basis.
  • Monitors and documents activities and participants’ progress through accurate, thorough, and timely documentation of client reports.
  • Maintains compliance with assigned contract(s) including: Meeting productivity requirements Maintaining minimum caseload requirements Timely and thorough completion of program reporting
  • Participates in regular supervision and ongoing professional development to evaluate and improve the quality of youth development services provided and ensure that services are culturally appropriate and equitable for clients.
  • Performs other duties as assigned.
