About The Position

The Care Manager will provide care management services to clients, which includes assessing and assisting with their needs and coordinating care in a warm and compassionate environment. This position requires following through and assisting clients with their plan of care, ensuring that those we serve receive the highest quality of service in a caring and compassionate atmosphere. The role is part-time, requiring approximately 8 hours per week, with working hours from 9:00 am to 5:00 pm, Monday to Friday.

Requirements

  • Bachelor's degree is required.
  • Ability to read, write, understand, and communicate effectively in English.
  • NJ driver’s license in good standing.
  • Possess an insured vehicle to travel to and from clients’ homes.
  • Ability to deal with others in a diplomatic, tactful, and confidential manner.
  • Ability to maintain composure under stress or in dealing with residents, staff, and families.
  • Flu vaccination required during flu season.
