The Internal Case Manager for Inpatient Rehabilitation, in collaboration with the multi-disciplinary team, coordinates the patient stay and discharge planning from the time of admission to the time of discharge. The Internal Case Manager supports the delivery of quality care in the most cost effective way by managing the human, material and financial resources needed for individual healthcare needs. The Internal Case Manager is also responsible for ensuring patient and patient's caregiver(s) education in preparation for discharge readiness.
