Care Manager RN Full- time Day

The RN Care Manager is responsible for carrying out effective case management functions for identified patients by planning, coordinating, negotiating, procuring and managing the care and needs of complex patients and their families throughout the continuum of care. Requirements
- Associate's Degree - Associate's Degree in Nursing or a nursing diploma upon hire.
- California Registered Nurse License upon hire.
- 2 years of acute care experience in a Medical Surgical or Inpatient setting.
- 1 year of experience in care management or utilization review in any setting, OR successful completion of TIPS program or Case Management Orientation Program upon hire.
Nice To Haves
- Bachelor's Degree in Nursing or higher within 3 years of hire.
- CCM or ACM upon hire.
