Riverside Healthcare is seeking a dedicated and compassionate Care Manager Registered Nurse to join our dynamic team. This role is integral in coordinating patient care, improving health outcomes, and ensuring a seamless transition through various levels of care. The ideal candidate will possess a blend of clinical expertise, strong organizational skills, and the ability to advocate for patients and their families. Additionally, this position requires flexibility with assignments within professional scope/duties/licensure. Essential Duties Patient Care Coordination: Develop, implement, and manage individualized care plans for patients with complex health needs. Collaborate with multidisciplinary teams to ensure comprehensive and effective care. Completes management functions including patient/family education. Refers reviewed cases to a Physician Advisor as needed. Assessment and Planning: Conduct thorough assessments of patients medical, psychological, and social needs. Create and adjust care plans based on ongoing evaluations and patient progress. Patient Advocacy: Serve as a primary contact for patients and their families. Provide education on health management and resources, ensuring patients understand their care plans and treatment options. Case Management: Monitor and evaluate patient care and outcomes, coordinating with healthcare providers to address any issues or changes in patient condition. Ensure timely follow-up and continuity of care. Documentation: Maintain accurate and up-to-date records of patient interactions, care plans, and outcomes. Ensure compliance with regulatory requirements and organizational policies. Resource Management: Identify and facilitate access to community resources and support services for patients. Assist in managing care costs and optimizing the use of healthcare resources. Quality Improvement: Participate in quality improvement initiatives to enhance patient care and optimize processes within the care management program.
