About The Position

The Care Manager is responsible for the overall tracking, management, and timely care provisions for patients with chronic medical conditions. This position works closely with on-site physician and mid-level providers to ensure chronically ill patients are seen on a routine and scheduled basis dictated by their disease process.

Requirements

  • Must be a graduate of an accredited school of nursing
  • Three (3) years of relevant clinical experience is required. Prior medical/surgical or emergency/trauma experience is preferred.
  • Currently licensed as a Registered Nurse in the state within which services are provided and possess and maintain current CPR certification.

Nice To Haves

  • Prior medical/surgical or emergency/trauma experience is preferred.
  • Previous corrections experience preferred.

Responsibilities

  • Maintain a comprehensive log for all chronically ill patients, including appointment schedules, diagnostic testing, and disease details.
  • Prepare patients for chronic care visits, ensuring all required testing is completed and results are available for the provider.
  • Assist patients during visits as directed by physicians or mid-level providers, provide relevant education, and ensure special needs are addressed on a patient-specific basis.
  • Schedule off-site specialty consultations, present recommendations to on-site providers, and ensure corresponding orders are fulfilled.
  • Maintain chronic care statistical data, review aftercare recommendations post-ER or hospitalization, and conduct medication compliance studies, ensuring all reports are submitted as requested by the Medical Director.
  • Collaborate with the provider team and Director of Nursing to optimize facility resources for chronic care patients and ensure compliance with National Commission on Correctional Health Care standards.
