Requirements

• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing.
• 2 - 4 years of related experience.
• RN - Registered Nurse - State Licensure and/or Compact State Licensure required.

Responsibilities

• Evaluate the needs of members and identify barriers to accessing appropriate care.
• Develop ongoing care plans and collaborate with providers to identify specialists and community resources.
• Identify problems/barriers to care and provide appropriate care management interventions.
• Coordinate between members, families, and care provider teams to ensure adequate person-centered care.
• Provide ongoing follow-up and monitoring of member status and progress towards care plan goals.
• Facilitate care management and collaborate with providers to ensure timely access to needed services.
• Perform outreach to assess member needs and collaborate with resources.
• Document and maintain member information and care management activities for compliance.
• Educate members and their families on disease processes and healthcare options.
• Provide feedback to leadership on opportunities to improve care delivery.
