About The Position

The Care Manager at CVS Health serves as a frontline advocate for members, focusing on assessing, planning, implementing, and coordinating case management activities to enhance overall wellness. This role is essential in evaluating the medical needs of members who may not be able to advocate for themselves, ensuring they receive the necessary support and resources for their health care journey.

Requirements

• Active and unrestricted RN licensure in the state of Michigan.
• Minimum 3+ years of clinical practical experience.
• Minimum 2+ years of experience in Care Management, discharge planning, or home health care coordination.
• Willingness and ability to travel up to 25%-50% in Wayne/Macomb counties.

Responsibilities

• Develop a proactive plan of care to enhance short and long-term outcomes for members.
• Conduct evaluations of member needs using clinical tools and data review.
• Incorporate strategies to reduce risk factors and address complex health and social indicators.
• Perform assessments considering various information sources, including claims, to address co-morbid conditions.
• Utilize a holistic approach to determine the need for referrals to clinical resources and interdisciplinary team members.
• Collaborate with supervisors and stakeholders to overcome barriers in achieving member goals.
• Present cases at interdisciplinary case conferences and utilize case management processes in compliance with regulations.
