About The Position

Corewell Health operates over 300 outpatient clinics across Michigan, bringing a multitude of care options to the communities that we serve. There are over 40 different types of primary care and specialty practices, which allows those who work in these clinics the opportunity to learn and explore various interests or to specialize in one area. The position involves planning and prioritizing care for individuals and populations of patients, focusing on strategies that will promote optimal health within populations. The role requires expertise and current knowledge in nursing care and management of a caseload of patients of varying complexity, aiming to improve patient, family, and health systems/community outcomes through educational concepts/skills and preventive care in a managed care environment. Responsibilities include assessing internal and external referrals, developing care plans, communicating with healthcare teams, measuring patient care outcomes, and documenting patient data according to department guidelines.

Requirements

  • Bachelor's Degree in Nursing.
  • State of Michigan RN License.
  • 5 years of relevant clinical experience preferred.
  • 2 years of relevant experience in clinical nursing and/or case management/managed care.

Responsibilities

  • Plans and prioritizes care for individuals and populations of patients.
  • Demonstrates expertise in nursing care and management of a caseload of patients.
  • Assesses internal and external referrals to identify patient needs and resources.
  • Develops plan of care and makes recommendations to healthcare providers.
  • Assesses educational needs of patients and develops appropriate teaching strategies.
  • Communicates and collaborates with patients, providers, and payers to coordinate services.
  • Measures patient care outcomes and analyzes data trends.
  • Documents patient data, plans, interventions, and outcomes according to guidelines.
