About The Position

The Care Manager for Project AWARE at RiverStone Health is responsible for providing care management to high-risk patients, particularly school-aged youth, to improve their health outcomes. This role involves collaborating with a multidisciplinary Patient Care Team to identify barriers to care, develop individualized care plans, and facilitate access to necessary health services. The position emphasizes patient engagement, advocacy, and coordination of care, particularly in the context of mental health and social services, as part of a grant-funded initiative in partnership with the Billings Public School District.

Requirements

  • High school diploma or equivalent.
  • Experience in a social work or medical setting.
  • Valid Montana driver's license.
  • Knowledge of HIPAA and confidentiality regulations.

Nice To Haves

  • Bachelor's degree in social work, sociology, human services, psychology, or a related field.
  • One or more years of medical office and/or electronic health record experience.
  • Experience in care management of youth and their families.

Responsibilities

  • Provide care management to high-risk patients by identifying those likely to benefit from effective care management.
  • Perform patient assessments and develop individual care plans based on comprehensive evaluations of medical, psychological, and social histories.
  • Maintain an active caseload and conduct follow-up calls to monitor patient symptoms and progress toward health goals.
  • Coordinate communication between patients, families, medical staff, and community resources to ensure comprehensive care.
  • Educate patients on health insurance options and assist with enrollment in relevant programs.
  • Act as an advocate for patients within the care team and community support services.
  • Utilize motivational interviewing techniques to support patients in following treatment plans and achieving their health goals.
  • Work collaboratively with the AWARE team to deliver comprehensive care and engage families in mental health initiatives.
