This position will be housed out of our HUB/Central office located at 555 Maryville University Drive. The Nurse Practitioner (NP) Care Manager provides service through core competencies of direct care, consultation, expert guidance, leadership, ethical decision making and collaboration. The NP Care Manager will use their advanced clinical skills and training to assess, diagnose, manage, and treat. Essential to the role is the ability to collaborate and participate in care coordination activities that meet the patient’s needs. These skills are essential to reducing barriers when managing discharge planning, directing transitions of care, and assisting patients throughout their health care journey. This role focuses on assessment, diagnosis, treatment, care coordination, and chronic disease management for patients in their homes or via telehealth platforms. The NP Care Manager works collaboratively with physicians, care management teams, home health agencies, and other healthcare professionals to ensure high-quality, timely, and accessible care.

Requirements

  • Master’s Degree of Science in Nursing
  • Licensed and certified as an Advanced Practice Nurse in Missouri
  • 2 or more years’ experience as an Adult, Family or Gerontology Nurse Practitioner, is preferred
