The Care Manager MSW provides professional, comprehensive patient centric care management services for at risk patients in an acute care environment. Accountabilities include assessment and planning, coordination of care, discharge planning, documentation of interventions, regulatory compliance and patient advocacy. The goal of the Inpatient Care Management MSW is to ensure the continuity of care for vulnerable patients by identifying needed resources to address social, financial, cognitive/behavioral or legal barriers to care access. Please note the following important details regarding this position: + This posting includes multiple openings for a Care Manager MSW + Available Shifts: + Full-Time, Day + This is an onsite position, requiring daily attendance at the designated location. + Candidates must reside within commutable distance of the following office location: + Covenant Children's Hospital, Texas: Lubbock + Must be available to work weekends and holidays as scheduled, based on operational and departmental requirements.

Requirements

• Master's Degree in Social work from an accredited CSWE program
• Upon hire: Texas Masters Social Worker License - Texas State Board of Social Worker Examiners
• 1 or more years of Social work experience in any healthcare setting
• 1 or more years of experience in acute care setting or successful completion of TIPs program or Case Management Orientation Program
• Must be available to work weekends and holidays as scheduled, based on operational and departmental requirements.
• Candidates must reside within commutable distance of the following office location: Covenant Children's Hospital, Texas: Lubbock
• This is an onsite position, requiring daily attendance at the designated location.
