About The Position

- The Care Manager III at LIFESTREAM BEHAVIORAL CENTER is responsible for overseeing and coordinating comprehensive care management services for clients with complex behavioral health needs. This role involves collaborating with multidisciplinary teams to ensure the delivery of high-quality care, improving client outcomes, and supporting clients in achieving their recovery goals.

Requirements

  • - Bachelor's degree in Social Work, Psychology, Nursing, or a related field from an accredited institution.
  • - Minimum of 5 years of experience in a care management role within a behavioral health setting.
  • - Proven track record of developing and implementing care plans for individuals with complex mental health needs.
  • - Experience in coordinating with multidisciplinary teams to ensure comprehensive care delivery.
  • - Familiarity with state and federal regulations related to behavioral health services.
  • - Strong analytical skills to assess client needs and develop effective care strategies.
  • - Excellent communication skills, both verbal and written, for interacting with clients, families, and healthcare professionals.
  • - Ability to work independently and manage multiple cases simultaneously with attention to detail.
  • - Proficiency in using electronic health record systems and other relevant software applications.
  • - Demonstrated ability to handle sensitive information with confidentiality and professionalism.
  • - Strong problem-solving skills and the ability to adapt to changing situations and environments.

Responsibilities

  • - Develop and implement individualized care plans based on thorough assessments of clients' needs, strengths, and preferences.
  • - Coordinate with healthcare providers, social services, and community resources to ensure seamless delivery of services and support for clients.
  • - Monitor and evaluate client progress, adjusting care plans as necessary to address changing needs and circumstances.
  • - Advocate for clients' needs and rights, ensuring they have access to appropriate services and resources.
  • - Provide education and support to clients and their families to enhance understanding of behavioral health conditions and treatment options.
  • - Maintain accurate and up-to-date documentation of all client interactions, assessments, and care plans in compliance with organizational and regulatory standards.
  • - Participate in multidisciplinary team meetings, case conferences, and training sessions to enhance professional development and improve service delivery.
  • - Collaborate with other care managers and staff to identify opportunities for process improvements and contribute to the development of best practices in care management.
  • - Ensure compliance with all relevant policies, procedures, and regulations related to care management and behavioral health services.
