About The Position

Requirements

• Master’s Degree in Social Work or related field
• Active DC LICSW license
• 3+ years care management or community health experience
• Strong communication and documentation skills
• Care coordination and case management
• Clinical and psychosocial assessment
• Community resource navigation
• Discharge planning and care transitions
• Electronic documentation systems
• Time management and organization
• Cultural competence

Nice To Haves

• Spanish preferred

Responsibilities

• Manage a caseload of members with moderate to complex needs
• Conduct assessments and community-based outreach visits
• Develop and monitor individualized care coordination plans
• Coordinate medical, behavioral health, and community services
• Support discharge planning and transitions of care
• Educate members and families
• Document care activities accurately
