About The Position

The Care Manager in the Emergency Department at Intermountain Health is responsible for overseeing daily care management activities in the Ambulatory/Community Setting. Requirements

  • Master's degree in Social Work from an accredited program or Bachelor's Degree in Nursing (BSN).
  • LCSW license or SWC/LSW required at hire, with LCSW required within 3 years of employment.
  • Three years of experience as a Care Manager in an acute care setting for Social Workers.
  • Current valid State RN License required for RN candidates.
  • Three years of experience as a Registered Nurse in an acute care setting.

Nice To Haves

  • Care Management Certification preferred.
  • At least five years of care management experience in an acute care setting.

Responsibilities

  • Coordinate care and services for patients needing assistance or meeting Care Management criteria.
  • Collect in-depth information about patients' medical, functional, and social conditions to develop tailored care plans.
  • Work with patients and families to establish specific goals and actions based on assessments.
  • Coordinate discharge planning and execute interventions to meet established goals.
  • Organize and integrate necessary resources to accomplish care goals.
  • Assess patients' prior functioning levels and access to community resources.
  • Conduct psychosocial needs assessments and interventions as per guidelines and licensure.
  • Assist the care team in developing comprehensive care plans, ensuring appropriateness of services and support.
  • Collaborate with physicians and healthcare team members on behalf of patients.
  • Identify admission and continued stay issues to enhance patient care quality.
  • Help identify strategies for reducing length of stay and optimizing service utilization.
  • Attend and participate in rounds and case conferences, advocating for patient rights.
