About The Position

Build your Career. Make a Difference. Presbyterian is hiring a skilled RN to provide direct patient care to an assigned group of patients in an Outpatient Clinic. Common responsibilities include preparing the patient for the visit, maintaining efficient patient flow, assisting the provider with treatments and procedures, medication administration, support for clerical functions to include scheduling, referrals, authorization and in-basket support with the EMR. Requirements

  • Associate’s degree in nursing
  • State of New Mexico or Compact State Nursing License
  • BLS certification REQUIRED at at time or hire
  • 1-3 years ambulatory experience

Responsibilities

  • Assessment of an identified patient population based on symptoms and medical history.
  • Development of a plan of care based on preventative care needs, using standard based practice guidelines.
  • Collaborates with patients to update problem list, current medication list and coordinates visit plan.
  • Serves as patient advocate.
  • Serves as communication link between Inpatient Care Manager, HHC Care Manager and PHP Case Manager.
  • Documentation within scope of practice.
  • Other duties as required
