About the position

The Care Manager in the Cardiac Cath Lab plays a crucial role in providing comprehensive care for individuals, families, and community groups. This position encompasses a wide range of responsibilities, from patient admission through to post-discharge care, ensuring that patients receive the necessary support throughout their healthcare journey. The Care Manager is equipped to operate in both structured and unstructured healthcare environments, which may present unique challenges and require independent decision-making skills. In unstructured settings, the Care Manager must navigate situations that lack established standards, demonstrating adaptability and critical thinking to meet patient needs effectively. In addition to direct patient care, the Care Manager is responsible for directing the work of staff nurses, unlicensed caregivers, and other healthcare professionals involved in patient care. This leadership role requires strong communication and organizational skills to coordinate care effectively and ensure that all team members are aligned in their approach to patient management. The Care Manager will also engage in ongoing assessment and evaluation of patient care plans, making necessary adjustments to optimize outcomes and enhance the patient experience. The position is based in the Cardiac Cath Lab at Englewood Health, a leading healthcare provider in New Jersey known for its commitment to clinical excellence and patient safety. The Care Manager will work full-time during day shifts, with hours from 6:00 am to 6:30 pm, including staggered starts and on-call responsibilities as needed.

Responsibilities

  • Provide comprehensive care management for patients from admission to post-discharge.
  • Direct the work of staff nurses and unlicensed caregivers in the Cardiac Cath Lab.
  • Assess and evaluate patient care plans, making adjustments as necessary.
  • Function effectively in both structured and unstructured healthcare settings.
  • Demonstrate independent decision-making in unstructured environments.
  • Coordinate care among various healthcare professionals to ensure optimal patient outcomes.

Requirements

  • A minimum of 2 years of Cath lab experience required.
  • BSN required; Master's degree preferred.
  • Current NJ RN License required.
  • BLS and ACLS certification required.
  • Specialty certification to be obtained within 6 months of eligibility.
