About The Position

The Care Manager is responsible for facilitating the process flow for Transitional and Complex Care Management within IMA Medical Group. This role focuses on providing high-quality primary care services to the elderly, ensuring that patients receive comprehensive and coordinated care tailored to their specific health needs. The Care Manager will work closely with an interdisciplinary team to implement care plans, conduct assessments, and support patients in managing their health conditions effectively.

Requirements

  • High school diploma or GED required; college coursework in Healthcare preferred.
  • One year of related experience in outpatient clinic, hospital setting, insurance, medical records, billing, or related area preferred.
  • Six to twelve months of data entry experience required.
  • Intermediate level MS Excel skills required.
  • Working knowledge of medical terminology preferred.
  • Experience with hospital portal systems preferred.
  • Effective interpersonal skills required.
  • Good organizational and time management skills required.
  • Effective communication skills (written and oral) required.
  • Ability to multitask and possess good phone skills required.
  • Detail-oriented and a team player required.
  • Bilingual in English and Spanish required.

Nice To Haves

  • Experience in outpatient clinic or hospital settings.
  • Knowledge of medical records and billing processes.

Responsibilities

  • Establish a systematic process to identify patients who may benefit from care management.
  • Implement a comprehensive approach to achieve desired outcomes for patients based on disease groups such as CHF, COPD, DM, and Cancer.
  • Perform telephonic Care Management, including patient clinical assessment and education.
  • Participate in the identification, collection, and analysis of data for potential enrollment in the Disease Management program.
  • Apply critical thinking and problem-solving techniques to the disease management process.
  • Conduct thorough and objective holistic assessments of patients' current status and needs.
  • Partner with physicians for effective clinical management and resource utilization.
  • Provide clinical direction and coordination to an interdisciplinary team to formulate and implement care plans.
  • Maintain knowledge of disease processes, resources, and treatment options appropriate to the patient population.
  • Document all activities according to clinical guidelines.
  • Demonstrate a positive attitude and understanding of care management objectives.
  • Exhibit excellent communication and interpersonal skills with various stakeholders.
  • Identify opportunities for performance improvement and participate in related activities.
  • Act as a liaison for patients as needed.
