About The Position

The Care Management Technician at Bayfront Health is responsible for supporting the care management team in facilitating patient transitions and throughput. This role involves educating patients and families about the care management process, coordinating with healthcare providers, and managing various administrative tasks to ensure efficient operations within the department.

Requirements

• High school diploma or GED.
• Two years of experience working with customers and/or patients, including one year in a health-related field.
• Working knowledge of Microsoft Office programs (Word, Excel, Outlook, PowerPoint).

Responsibilities

• Educates patients/families on the roles of the care management team and ensures timely transitions to the next level of care.
• Assists with Care Management requests, including arranging nursing home returns and contacting facilities to ascertain bed availability.
• Identifies availability of community resources and enrolls patients in assistance programs.
• Refers patients to community agencies and arranges transportation as needed.
• Obtains authorization numbers from payors and necessary contact information for follow-up.
• Issues Important Message from Medicare (IMM) letters and other notifications as required.
• Maintains office functions including mail management, phone coverage, and equipment maintenance.
• Maintains required files, statistics, and reports for accurate information access.
• Assists in scheduling and coordinating meetings for the department.
• Participates in quality and safety initiatives to improve practice standards.
• Prioritizes workload to manage multiple tasks effectively.
