The Care Management Support Coordinator II at Fidelis Care plays a crucial role in supporting administrative care management activities.   • Perform outreach to members via phone to support care plan next steps and address questions or concerns.
  • Serve as a point of contact for members, providers, and staff to resolve issues and document member records.
  • Support member onboarding and day-to-day administrative duties, including sending welcome letters and educational materials.
  • Document and maintain non-clinical member records in accordance with state and regulatory requirements.
  • Provide information on existing benefits and resources, making referrals to address Social Determinants of Health (SDOH) needs.
  • Explain care plan procedures and protocols to members and providers.
  • Assist members in connecting to health plan and community resources for high-quality customer care.
  • High School diploma or GED required.
  • 1 - 2 years of related experience preferred.
