About The Position
The Care Management Program Specialist ensures the successful on-boarding of new providers and patients as they implement Chronic Care Management, Transitional Care Management, and evolving CMS Care Coordination or other programs for chronic care. Onboarding is the most important phase of the client life cycle, and the Care Management Program Specialist is responsible for building and maintaining the client's satisfaction.
