About The Position

The Care Management (CM) Nurse will participate in direct patient care, relaying information from the providers to the patients, and answer patient questions in a manner respectful to the patient. The nurses will assist the providers as well as other staff members, in ensuring that our patients receive the highest level of care possible. The CM nurse will have the responsibility of contacting patients for their non-face-to-face visits monthly, between their on-site provider appointments, evaluating telephone calls, and provide sound medical advice and consultation to the patient as appropriate and as directed by the provider’s care plan.

Requirements

  • Good computer skills; Outlook, Windows, Microsoft Office applications
  • The CM nurse will be trained in our current EMR, Athena.
  • Good time management skills, self-motivating.

  • Current valid Indiana nursing license
  • 2 to 3 year of clinical experience in a medical office; phone triage experience preferred
  • BLS-trained. To be achieved within three months of hire date.

Responsibilities

  • Help create and maintain an atmosphere of professionalism, respect, and safety for all patients and staff.
  • Contact patients by telephone for non-face-to-face visits in between provider appointments.
  • Schedule same-day appointments, as required.
  • Provide patient education, information and teaching.
  • Trains in the use of equipment used within the Health Center, such as:  nebulizers, cholesterol machines and HgbA1C machines, autoclave. This list is not meant to be exhaustive.
  • Assists in training patients in the use of home monitors, such as blood pressure monitors, Hemoglobin A1C meters, etc.
  • Administers injections, immunizations, and treatments.
  • Obtains blood samples by finger stick or venipuncture.
  • Prepares the patient for examination by performing “Intake” within the Electronic Medical Record (EMR): recording vital signs, current medications on med list correctly, Quality Measures checked, concise description of reason for today's visit. Verify Allergies, demographic information, as needed.
  • Assists the providers as requested and required.
  • Assists with referrals, prior authorizations and making specialty or test appointments. Knowledge of test requirements, the information provided by the test results, and translates in a manner corresponding to the patient’s literacy level. The CM nurse must be able to explain the above thoroughly and clearly, ensuring that all the patients’ questions have been answered.
  • Relay information and messages to and from providers as necessary
  • Completes patient forms, in a timely fashion, and present these to the providers for signature, as required signatures
  • Knowledgeable in proper handling and disposal of all medical waste materials.
  • Assists the Clinical Support Supervisor with staffing issues.
  • Answer patient phone calls relating to illness, gather pertinent information regarding illness the physician will need for an informed decision/ treatment. The provider’s response will then be documented in the patient record and respectfully explained to patient calling.
  • Assist with clinical related questions for ER and IP follow up patients.
  • Mitigate ER Utilization by encouraging care management and primary care provider appointments.
  • Monitor clinical quality measures for patients by explaining preventative measures to patients and collaborating with the Utilization Management Coordinator, Managed Care Coordinators and Utilization Review Nurse manager.
  • Strategize and prioritize best practices for closing care gaps.
