About The Position

The Care Management Nurse at San Francisco Health Plan (SFHP) plays a crucial role in providing medical care management to members assigned to care management services. Reporting to the Supervisor of Care Management, this position involves co-managing cases alongside Care Management Community Coordinators (CMCC) and requires a blend of office, field, and remote work. The nurse will be responsible for conducting medical assessments, providing chronic condition education aimed at self-management, and performing medication reconciliation. The role also includes ongoing medical and nursing consultation to the CMCC, ensuring that all medical needs of the members are addressed effectively. In this position, the Care Management Nurse will coordinate care management activities with the assigned CMCC, managing up to 50 cases at any given time. The nurse will perform care management within the scope of their licensure for members with complex and chronic care needs. This includes providing nursing input on co-managed cases, conducting medical assessments to inform the creation of member-centered care plans, and coordinating with various medical teams to ensure members have access to necessary services. The nurse will also monitor and update nursing care plan goals and interventions, focusing on chronic condition management and health coaching education to empower members in their self-management skills. The role requires a proactive approach to care coordination across different settings, helping clients and caregivers understand their healthcare options. The Care Management Nurse will also engage in community-based visits, which necessitates a successful completion of a sex offender registry screen due to the nature of the work. This position is vital in supporting the health and well-being of SFHP members, particularly those from underserved populations.
