About The Position
The Care Management Manager at White Plains Hospital is responsible for ensuring the delivery of high-quality services in accordance with established hospital, departmental, and regulatory/accreditation agency standards. This role encompasses the overall operations of the Care Management department, working closely with the Utilization Management (UM) manager and the department director. The manager is accountable for developing departmental goals and objectives that align with the hospital's mission, managing personnel, and overseeing the preparation and monitoring of departmental budgets. In this position, the manager ensures compliance with hospital policies and procedures, including those related to employee orientation, skill competency, and continuing education requirements. The manager actively participates in the hospital's Performance Improvement Program, setting up departmental plans based on regulatory requirements and addressing areas identified for improvement. The role also involves maintaining a safe environment for both employees and patients, following up on incidents and safety issues, and fostering effective communication and collaboration within the interdisciplinary team at the hospital. The Care Management Manager serves as a role model and change agent, promoting positive customer service skills and employee relations. This position requires establishing and maintaining relationships with community agencies to ensure optimal care for patients post-discharge. A strong understanding of Medicare and Medicaid guidelines, as well as knowledge of HMO operations and private health insurance, is essential for compliance and effective management of the department.
