Provides coordinated care for Emergency Room patients who are at high risk for readmission. This involves identifying and securing services for those anticipated to have difficulty meeting their care needs. Assist patient and family to create a smooth transition with specific interventions for high risk situations. Makes telephone contact with these high-risk patients within 24-48 hours after discharge to evaluate patient education and determine patient's compliance with discharge instructions. Responds to difficult service situations and supports families and staff. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.
