Job Highlights: Supervising care managers serving members with an I/DD or a TBI must have one of the following minimum qualifications: A bachelor’s degree in a human services (or other) field and five years of experience providing care management, case management, or care coordination to complex individuals with I/DD or TBI A master’s degree in a human services field and three years of experience providing care management, case management, or care coordination to complex individuals with an I/DD or a TBI This Opportunity: The primary responsibilities of the Care Management Director are to provide clinical, administrative, and operations supervision and guidance to the Care Management team to achieve key goals of BH I/DD Tailored Plan.
