About The Position

At AltaMed, the Care Management Coordinator provides essential support to the pharmacist-led chronic disease management program. This role is crucial for assisting with both chronic disease management and medication therapy management. Key responsibilities include reviewing registry data and other data sources to identify patients who are eligible for chronic disease management, based on established guidelines. The coordinator is also responsible for performing outreach to enroll these appropriate patients into the program. Requirements

High School diploma

Nice To Haves

Pharmacy technician certification
2 years of clinical experience
Bilingual English/Spanish
