About The Position

The Care Management Coordinator provides essential clerical support for the Care Management Department. Under the supervision of licensed clinical staff, accountabilities include coordinating communication between patients, health care providers, insurance companies and other stakeholders to ensure safe and efficient transitions of care, payment for clinical services provided and regulatory compliance. Requirements

• 2 years - Administrative experience, healthcare setting preferred or demonstrated knowledge of healthcare terminology

• Associate's Degree - Business, Healthcare or related field
