About The Position

The Care Management department employs a portfolio of programs to promote access to high-quality healthcare and social services with the goal of improving health outcomes and reducing preventable healthcare costs. This position will collaborate closely with peers and management to support various programs and initiatives through administrative tasks, coordination, task management, as well as outreach and engagement with members. Duties include, but are not limited to, complex visit scheduling, telephonic outreach, reviewing health records, coordinating care and services, and compiling and maintaining data to support program operations and evaluation.

Requirements

  • Bachelor’s or associates degree preferred
  • At least 2 years prior full-time work experience (healthcare not required, but preferred).
  • Extremely strong verbal and written skill set. Ability to write high-level communications to executives.
  • Strong quantitative skill set, including basic statistical knowledge, and intermediate to advanced mathematics knowledge.
  • Ability to utilize advanced Microsoft Office Skills in projects, including Microsoft Excel graphing features, Microsoft Vizio flow charts, and Microsoft Word.
  • Ability to perform any administrative duties such as scheduling, preparing training rooms, ordering team lunches, arranging group outings, and attending to other management needs.

Responsibilities

  • Identify members eligible for various programs and services available within the Department.
  • Conduct telephonic outreach to introduce programs and services and solicit participation from eligible members.
  • Schedule appointments for supportive programs such as transportation assistance and high-risk clinic visits.
  • Manage appointment scheduling for field-based staff.
  • Monitor and respond to incoming requests.
  • Maintains accurate records of program activities by collecting and inputting data into various program evaluation tracking systems.
  • Manage department supplies, inventory and receipt of marketing materials, as needed.
  • Review of program data for completeness and accuracy for program evaluation purposes.
  • Maintain a high level of organization
  • Meet company and departmental goals, and submit timely and high-quality deliverables to department leadership.
  • All other duties as directed by management.
