About The Position

The Care Management Consultant I at Genesis HealthCare System plays a crucial role in assisting patients through the healthcare system by acting as a patient advocate and navigator. This position involves creating personalized care plans, coordinating with healthcare providers, and utilizing data analytics to manage patient health effectively. The consultant is responsible for ensuring continuity of care, improving patient outcomes, and preventing readmissions through effective communication and collaboration across various care settings.

Requirements

  • Current Ohio RN licensure, Licensed Social Worker (LSW), or Counselor (LPC) in the State of Ohio.
  • If RN, must have BSN with 3 years of clinical RN experience or ADN with 7 years of clinical RN experience.
  • If LSW or LPC, Bachelor's degree in Social Work is required.
  • Strong analytical, data management, and computer skills.
  • Current knowledge of discharge planning, utilization management, case management, and managed care reimbursement.

Nice To Haves

  • Experience in motivational interviewing and self-management support strategies.
  • Familiarity with predictive analysis and risk stratification techniques.

Responsibilities

  • Conduct assessments of patient/family needs and formulate documented care plans for high-risk patients.
  • Coordinate continuity of patient care following hospital admissions, discharges, and ER visits.
  • Delegate care tasks while ensuring accountability for patient outcomes and supporting assistive personnel.
  • Lead and coordinate interdisciplinary treatment teams to make clinical and benefit decisions for high-risk patients.
  • Identify appropriate providers and facilities, maintaining positive relationships with patients, families, and providers.
  • Manage care across multiple settings to ensure appropriate utilization of resources and improve patient care quality.
  • Guide the development of healthcare partnerships to enhance health outcomes.
  • Ensure transitions of care are completed and that patients have the necessary resources to improve outcomes and prevent readmissions.
  • Manage high-risk patient care using data analytics and patient registries.
  • Participate in data collection, health outcomes reporting, and clinical audits to evaluate care effectiveness.
