About The Position

INTEGRIS Health Southwest Medical Center, Oklahoma's largest not-for-profit health system has a great opportunity for a Care Management Associate in Oklahoma City, OK. In this position, you'll work 8a-4p Monday - Friday OR 4-10-hour shifts and rotating weekends with our Southwest team providing exceptional care to those who have entrusted INTEGRIS Health with their healthcare needs. The Case Management Associate educates patients, families, and staff on the benefits and availability of home health care, hospice, skill nursing facilities, nursing homes, rehabilitation services, and home infusion services. Coordinates care in homebound service areas. Adheres to National Patient Safety Goals as appropriate based on the level of patient contact this position requires. Case Management Only: Completes the plan for discharge after the Social Worker or RN Case Manager have assessed and completed recommendations for the plan of care. Coordinates receipt of appropriate documentation and arranges transportation to support a smooth transition. Requirements

  • Bachelor's degree in health related field OR Bachelor's degree in business field with 2 additional years of healthcare experience OR Associate degree with 2 years experience in a variety of clinical settings OR License Practical Nurse (LPN) with 3 years of nursing experience OR 6 years of directly related experience in lieu of degree
  • LPN (Licensed Practical Nurse) Current licensure as a Licensed Practical Nurse (LPN) in the State of Oklahoma or current multistate license from a Nurse Licensure Compact (eNLC) member state OR Bachelor's degree in health related field OR Bachelor's degree in business field with 2 additional years of healthcare experience OR Associate degree with 2 years experience in a variety of clinical settings in lieu of LPN license
  • BLS (Basic Life Support) Issued by American Red Cross or American Heart Association within 30 days of hire
  • Must be able to communicate effectively in English (verbal/written).

Nice To Haves

  • 2 years experience in home health
  • Experience in healthcare education, marketing, public relations and/or communications

Responsibilities

  • Identifies potential candidates who could benefit from homebound services.
  • Educates the patient/family prior to discharge regarding home health care, skilled nursing, nursing home, rehab, hospice and home infusion services that are available.
  • Facilitates effective communication regarding the qualifying criteria for admission to various venues for post -acute hospital care.
  • Documents the referral in the medical record according to departmental policy.
  • Serves as a resource to the Physician, Case Manager, Social Worker and interdisciplinary team regarding post-acute care needs
  • Educates patients on standards of quality for patient satisfaction measures.
  • Keeps constant communication between the patient, family, RN Case Manager and the Social Work Team as to the status of all pending discharge arrangements.
  • Responsible for communication of appropriate medical records to receiving agency/facility and to the appropriate payer.
  • Leads age and developmentally appropriate patient education groups to address the emotional, physical and environmental needs of Behavioral Health and/or Addiction Recovery patients.
  • Participates in the writing of treatment plans with other program staff and physicians.
  • Assist in utilization review process by contacting patients? insurance companies to provide clinical information to authorize patient treatment.
  • Serves as a patient advocate in the patients? needs, as well as those individuals involved with patient.
  • Completes or facilitates the completion of Emergency detention paperwork, as well as ensure its distribution and receipt by legally designated individuals.
  • Processes orders/refills or laboratory for ordering physician.
  • Monitors patients? medications and provide education related to use and side effects.
  • Monitors vital signs as indicated by physician orders or if deemed medically necessary.
  • Provides referrals options for patient?s who are assessed, but do not enter the program.
  • Completes a comprehensive Initial Mental Health Assessment of patients clinical, psychological and financial needs utilizing all available resources.
  • Attends and participates in treatment team.
