Position Summary: This work at home role as a Care Management Assistant, you will support the coordination of member healthcare services by assisting with intake, screening, and triage of care team tasks, while helping ensure that members receive timely, appropriate care. You’ll interact with members, providers, and internal teams to gather information, provide updates, and facilitate smooth communication throughout the care process. Your role includes maintaining accurate and compliant documentation within internal systems, conducting non‑clinical research to support case activity, and monitoring cases for needed follow‑up or program referrals. You will also uphold quality and regulatory standards by adhering to company policies, protecting member confidentiality, and supporting compliance with care management guidelines and accreditation requirements.

Requirements

  • Call Center Experience
  • Strong verbal and written communication skills with the ability to handle member and provider interactions professionally.
  • Solid customer service skills, including problem‑solving, attention to detail, and a supportive, solutions‑focused approach.
  • Ability to stay organized, manage multiple tasks, and work effectively in a fast‑paced, team‑oriented environment.
  • Familiarity with basic medical terminology and general concepts used in care management (or willingness to learn).
  • Proficiency with computers, including navigating multiple systems and using Microsoft Word and Excel.
  • Ability to collaborate with a multidisciplinary care team, including clinical and non‑clinical staff.
  • High School Diploma
