InnovaCare Management Services Company, LLC is seeking an individual to provide administrative support to case managers and clinic teams. The role involves reviewing daily schedules to identify high-cost patients, participating in daily clinic morning huddles, and engaging in weekly UM/CM rounds. The individual will perform task-driven outreach according to the plan of care, schedule follow-up appointments after ED events, and send letters/reminders for patients not seen or not established. Additionally, the role includes following up with other teams for pending task completeness and providing Tier Two patient outreach for short-term case management. Responsibilities also include following up on referral/DME/specialist visits completeness with patients, assisting with required paperwork from hospitals, SNFs, LTAC, and home health, and conducting tuck-in calls to real-time rising risk patients. The individual will assist case managers and clinic staff with daily outreach for specific tasks, confirming medication adherence, attendance at specialist appointments, and the receipt of DME, HHC, or any resources outlined in the plan of care.

  • College degree or completion of college courses in healthcare, care management, or similar focus
  • Prior experience as a Medical Assistant, health coaching, or care management preferred
  • Good interpersonal skills
  • Ability to stay in constant communication with the team
  • Ability to multitask in a fast-paced environment
  • Bilingual in English/Spanish preferred but not required
