About the position
As a member of the multidisciplinary team, the Care Management Assistant provides discharge planning support to the Care Manager and Social Worker and supports patients and families / caregivers by reducing non-clinical barriers to discharge in the Inpatient and ED environments.
Responsibilities
- Provide discharge planning support to the Care Manager and Social Worker.
- Assist patients and families/caregivers in reducing non-clinical barriers to discharge.
- Collaborate with multidisciplinary team members to ensure effective discharge planning.
Requirements
- High School diploma or equivalent (GED).
- Two (2) years of experience in a healthcare or social services setting.
Nice-to-haves
- Associate's degree or two years of college in human services/health related discipline preferred.
- Knowledge of community-based/post-acute care resources such as primary care, skilled nursing facilities, home health, durable medical equipment, and transportation.
