About The Position

The Care Management Assistant (CMA) position at Denver Health is designed to provide paraprofessional support within the Care Management Department, focusing on discharge planning for patients. The CMA will coordinate post-discharge services in collaboration with Nurse Care Coordinators and Clinical Social Workers, ensuring smooth transitions of care and effective communication with external providers. This role also includes general office administrative duties and interim support for DME coordination.

Requirements

  • Bachelor's Degree in social science or healthcare field required, or a High School Diploma/GED with four years of experience in healthcare-related fields.
  • 1-3 years of experience in healthcare-related fields such as hospitals, home health providers, insurance utilization review, or case management.

Nice To Haves

  • Knowledge of third-party payor requirements and regulations including Medicare/Medicaid and commercial insurance.
  • Experience with data management tools such as Microsoft and Epic.

Responsibilities

  • Manage appropriate transitions of care for patients from the Emergency Department and inpatient settings, coordinating with Nurse Care Coordinators and Clinical Social Workers.
  • Serve as a contact for external providers, conducting follow-up calls to confirm referral receipt and obtain disposition regarding post-discharge referrals.
  • Conduct research to identify providers for specific services needed by patients, reviewing organizations for partnerships with insurance companies and cultural specialties.
  • Follow the organization's release of information policies to electronically send pertinent information for each referral type as directed.
  • Schedule follow-up appointments to support post-discharge care, including physician visits and other necessary services.
  • Perform other clerical duties and provide interim support for DME coordination.
