The Care Guide position is responsible for leading and coaching the CNA and Nurse teams at Woodland Park Houses, including communication and healthy conflict management. Recruitment, interviewing, and retention of staff are key responsibilities, as is providing support for CNA and Nurse orientation, and their continued growth. Serves as the direct supervisor (Guide) to the self-managed work team, CNA and Nurses, and is responsible for coaching, directing, and monitoring individuals/team, directly or through team coordinators. Monitors and evaluates the outcomes of the CNA and Nurses’ work as well as the quality of services and the quality of life of the residents in Woodland Park. Responsible for the completion of audits (blood sugar, vaccines, TB, anticoagulant monitoring, lab Kardex, bowel and bladder, catheter, etc.). Responsible for admission management, including order entry and admission assessments. Leads the pre-admission assessment process to include briefing, coaching, and preparation of the CNA and Clinical Support Team nurses. Provides education/leadership of transitions, including grief support. Leads investigations of accidents and incidents, including initial reports and collection of witness statements. QA/PI initiative follow-up to include charting and coaching Performance Improvement Plans (PIP’s). Works collaboratively with supportive living and marketing service lines to establish system-wide delivery that supports VMRC's mission, vision, and values.
