About The Position

The Care Guide, RN at Spira Care plays a crucial role in enhancing member experiences by managing interactions and assisting members in navigating the complexities of healthcare. This position emphasizes a collaborative, patient-centered approach, allowing healthcare professionals to make a significant impact on patients' lives. The Care Guide will engage with members primarily in face-to-face settings and over the phone, providing a seamless healthcare experience through a blend of administrative, educational, and care coordination functions.

Requirements

  • Associate or bachelor's degree in nursing.
  • Valid and active Registered Nurse (RN) license in Kansas and Missouri or multi-state compact license.
  • BLS certification from American Red Cross or American Heart Association.
  • 5 years of healthcare experience.
  • 3 years of full-time direct patient care and clinical experience in medical/surgical, pediatrics, or obstetrics.
  • Intermediate level knowledge of Clinical Guidelines required.
  • Previous experience in customer service or serving customers in some direct capacity.

Nice To Haves

  • Bachelor's degree in nursing.
  • Case Management Certification.
  • 5 years' experience in Care/Case/Disease Management, Health Plan, or Utilization Review.
  • Understanding of the Patient Centered Medical Home (PCMH) / Primary Care First (PCF) model.
  • Thorough knowledge of URAC and NCQA guidelines and state and federal regulations.

Responsibilities

  • Establishes a trusted partnership with patients in both face-to-face and phone settings.
  • Completes patient assessments by collecting and analyzing information and developing individualized care plans.
  • Monitors the effectiveness of care plans and intervenes as appropriate to improve health outcomes.
  • Provides high-quality customer service in interactions with patients, providers, and community organizations.
  • Acts as a liaison between patients, employers, providers, and support systems.
  • Utilizes effective communication to maximize collaborative relationships.
  • Manages projects and competing priorities to ensure timely information delivery to patients and relevant entities.
  • Builds relationships within the Care Center and shares best practices across other Care Centers.
  • Educates and works with patients at different educational and health literacy levels in a culturally sensitive manner.
  • Maintains confidentiality of all regulated information in compliance with laws and policies.
