About the position

At Devereux as a Care Coordinator you will impact the individuals you are working with by ensuring quality services are delivered, and all team members, family members and external stakeholders are aware of the students' progress in treatment. The Care Coordinator is a full-time position working within a Psychiatric Residential Treatment Facility (PRTF) for Children ages 8-21 years with behavior and emotional support needs, located in West Chester, PA.

Responsibilities

  • Welcome families/guardians to the Devereux Family, relaying expectations for family engagement/involvement while the child is in care and completing a Bio-psychosocial assessment.
  • Communicate with families/guardians the treatment and services being provided to their child.
  • Participate in client treatment team meetings and report on family engagement, supervised family visitation, family court, and care coordination activities that facilitate current treatment and discharge planning.
  • Complete documentation related to family communication/engagement and care coordination activities, and discharge summaries.

Requirements

  • Bachelor's degree in Social Work or related discipline required.
  • Six months experience in Social Service preferred.
  • Proficient computer skills, mastery of Microsoft Office Applications.
  • Experience with data collection/analyzing platforms (Radar, Pro-Filer, and/or other internal and external software applications pertinent to the position).
  • Strong organizational skills.
  • Excellent oral and written communication skills.
  • Ability to work independently, prioritize, manage multiple tasks simultaneously, and adhere to established guidelines.
  • Valid driver's license must be maintained.
