About The Position

The Care Coordinator at Permiacare plays a crucial role in supporting behavioral health patients by ensuring they have access to the necessary resources and tools to improve their mental health. This position involves collaborating with patients, doctors, and clinic staff to facilitate high-quality physical and behavioral healthcare, ultimately aiding patients in their wellness and recovery journey. The Care Coordinator is responsible for conducting screenings, assessments, and intake documentation to enroll patients in services, as well as linking them to community resources and providing follow-up support to achieve their recovery goals.

Requirements

• Bachelor's degree from an accredited college or university in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistance, gerontology, special education, educational psychology, early childhood education, or related fields.
• At least 1 year of experience in the mental health field preferred.
• Valid Texas Driver's license and auto liability insurance with an acceptable driving record.
• Ability to pass criminal history and background checks, as well as a pre-employment drug screen.
• Must obtain QMHP certification within 6 months of hire.

Responsibilities

• Work closely with behavioral health patients, doctors, and clinic staff to ensure access to necessary resources.
• Conduct screenings, assessments, and intake documentation for patient enrollment in services.
• Link patients to various community services to close the treatment gap.
• Provide follow-up support to ensure patients receive services and achieve recovery goals.
• Report major activities through periodic meetings while working independently under limited supervision.
