About The Position

The Care Coordinator plays a crucial role in the NorthStar Care Community, serving as the first point of contact for patients and families seeking hospice care. This position requires empathy, professionalism, and a strong understanding of hospice services, ensuring effective communication and coordination of care among various stakeholders.

Requirements

  • Associate's degree in a business-related or social services field or a minimum of five years of full-time healthcare, hospice care, or business operations work required; Bachelor's Degree preferred.
  • A minimum of two years of telephone-related experience required; experience in a healthcare environment preferred.
  • Strong proficiency in Microsoft Office products, especially Excel spreadsheets and database management.
  • Excellent interpersonal skills, sound judgment, and effective organizational skills.
  • Ability to maintain confidentiality and promote positive relationships at all levels.
  • Must be able to read, write, and speak English effectively for all essential job functions.
  • Familiarity with standard office business machines and ability to coordinate multiple tasks simultaneously.

  • Experience in a healthcare environment is highly preferred.

Responsibilities

  • Communicates and interacts with guests, customers, and employees in a courteous and professional manner.
  • Understands and communicates hospice philosophy and services clearly to callers.
  • Processes and enters referrals during high call volumes and after hours as needed.
  • Schedules admission visits and collaborates with triage RNs to manage patient calls and dispatch field staff.
  • Updates and manages the content of the Whiteboard for patient visits and comfort calls.
  • Assists triage RNs with daily reports and communicates with partner staff for shift reports.
  • Navigates multiple electronic medical records to assist patients and families effectively.
  • Enters information into electronic documentation systems in a timely manner.
  • Responds proactively to crisis calls and resolves situations for positive outcomes.
  • Reviews and analyzes patient files and incoming paperwork daily.
  • Documents conversations with patients, families, and staff in the electronic patient record system.
  • Addresses external customer inquiries and service delivery issues in collaboration with operations managers.
  • Coordinates communication for care provision and scheduling with interdisciplinary team staff.
  • Processes incoming and outgoing mail efficiently.
