About The Position

The Care Coordinator needs to demonstrate initiative, confidence, and a focus on making positive connections to coordinate care management services for the Behavioral Health Home clients. This role involves integrating, overseeing, and monitoring all aspects of the physical and behavioral health needs of Behavioral Health Home clients. The Care Coordinator will complete comprehensive assessments, develop and monitor interdisciplinary plans of care aimed at achieving stability within the community, and provide referrals and linkages to community services. Regular communication with other care providers to exchange information and identify needs is essential. The role also includes providing outreach to those in need but not currently receiving services and coordinating with managed care companies. Additionally, the Care Coordinator will provide transportation to clients using the agency’s vehicles and promote a culture of inclusion and belonging.

Requirements

  • BA/BS in human services.
  • Minimum of 2 years in human services.
  • 1 year in case management, including assessment of client needs, care planning, implementation of care plan, and regular review.
  • Demonstrated ability to function as a member of an interdisciplinary team.
  • Knowledge of mental illness/behavioral health, substance abuse, physical/medical health, and homelessness.
  • 1 year experience working in a health care setting providing direct care to clients is preferred.
  • Valid and insurable driver's license.
